I already provided my dependent's emergency information last year. Why am I getting a new request?

At the end of every sports season or school year, your dependent's record is disconnected from the organization as per our terms of use and privacy policy.

This means that every year or sports season, you'll be asked to confirm that all the information is still up-to-date and then to share this information for the current season.

If you entered this information last year, it will be pre-populated in all of the steps. Only new questions and waivers (Informed Consent) will need to be answered. If nothing has changed, it will take you less than 30 seconds to complete the form in a subsequent year.

Simply select your dependent, and click on Continue on each step until you get to the final Complete Info step, where you'll need to Share with the organization to give them access to your dependent's record for this year/season.
